Mental Health Disability Specialist will work as part of an integrated multidisciplinary team that provides comprehensive mental health and clinical case management to Clients over the age of 18. S/he will provide targeted and intensive clinical case management services with an emphasis on mental health disabilities as well as psychotherapeutic services. The Clinician will interface with clinical social workers, volunteers, and staff within the agency.Qualifications:

  • Master’s Degree in social work or related field 
  • Registered with the California Board of Behavioral Sciences or the Board of Psychology, Licensed preferred
  • Unrestricted CA Driver’s License and proof of auto insurance
  • Prior experience working with the LGBTQ community is desired
  • Bilingual Russian, Farsi, and or Spanish is a plus.
  • Knowledge of DMH Guidelines documentation requirements, and statistical reporting
  • Experience working with seniors, caregivers and mentally and physically disabled clients is highly desired
  • Excellent verbal and written communication skills
  • Excellent clinical judgement
  • Proficiency in common word processing, database software applications and the ability to learn Electronic Health Records Systems and new computer applications
  • Be culturally sensitive and able to work with a multicultural client population
  • Knowledge of community resources and the aging process
  • Knowledge of safety issues when working with at-risk clients
  • Ability to work collaboratively as part of an integrated team
  • Ability to maintain professional boundaries
  • Ability to utilize a strength based, non- judgmental, and Person-Centered Approach
  • Flexibility and ability to work autonomously
  • Stellar time management skills required
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

Responsibilities/Essential Functions:

  • Provide mental health office and field-based services to physically and mentally disabled clients
  • Conduct in-depth intakes and assessment covering psychological, social, physical, and environmental concerns and develop individualized treatment plans for clients
  • Provide psychotherapeutic services to individuals and groups in non-traditional community-based settings
  • Participate in multi-disciplinary team meetings to develop, coordinate, and monitor treatment plans and clients
  • Provide case management, advocacy, information and referral, coordination of benefits to disabled adults and seniors residing in service areas
  • Identify and develop family and other informal support systems for the client
  • Provide Crisis intervention services as needed
  • Provide clinical consultation to agencies and constituents of the community as appropriate.
  • Ensure meeting monthly direct service contacts to meet the agency contract expectation.
  • Participate in monthly staff meetings, weekly supervision, group supervision, and treatment planning meetings
  • Provide timely documentation and oversee chart recordkeeping as per funding source contract standards and requirements
  • Participate in staff trainings as needed, and complete a DMH Evidence Based Practice certification
  • Collaborate with DMH and the local municipality of the City of West Hollywood for the coordination of services for clients
  • Participate in utilization review of cases and other quality assurance activities
  • Provide outreach to the West Hollywood community, presentations, participation in collaborative service provider meetings and community events
  • Conduct in home assessments to provide resources and concrete services
  • Prepare monthly statistical report for funding sources
  • May provide direct services to seniors and families offsite at other JFS locations
  • May be required to be a Field Work Supervisor and/or preceptor for Social Work students
  • Other related responsibilities as assigned.
