The Home Safe Program Social Worker I is responsible for providing housing-focused case management services to older adults age 60+ and dependent adults 18-59 referred by APS who are experiencing homelessness and/or at imminent risk of becoming homeless. The Social Worker I will provide housing stabilization services that include home visits; information and referrals; a minimum of one (1) monthly face-to-face meeting with each client; assistance with budgeting and/or money management; assistance with accessing County and other mainstream benefits and services; developing a Housing and Services Plan; and provide financial assistance to clients as needed to identify and maintain housing.  QUALIFICATIONS:          

  • BA degrees in social work or a related social services or health related field.
  • Prior experience in providing case management assessments, housing plans and interventions preferred. 
  • Case management and community-based service delivery experience with the older adult population preferred.
  • Knowledge of community resources, housing subsidies, housing resources, and landlord/tenant rights.
  • HMIS or other database management strongly preferred. 
  • Experience conducting field based in-home visits and treatment strongly preferred.
  • Excellent communication skills both verbal and written and ability to utilize telehealth platforms.
  • Must be computer literate in MS Office applications.
  • Candidates must have a demonstrated ability to be culturally sensitive and able to work with a multicultural client/customer population; ability to work in a non-judgmental manner and foster positive outcomes; able to work as part of a team and maintain professional boundaries.
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment.
  • Current unrestricted CA Driver’s License and proof of auto insurance.

 RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

  • Provide housing focused case management services to promote long-term housing stability. 
  • Collaborate with each client to develop an individualized housing plan and budget to help clients understand their responsibilities and how to reduce costs to help maintain their housing. 
  • Conduct bi-weekly case management meetings with clients to achieve goals identified in the Housing and Services Plan; meeting locations will vary and may include clients housing units, encampments, shelters, or community agencies.
  • Maintain current and accurate documentation in Evolv and HMIS; case notes must be entered within two business days following the provision of services.
  • Ensure compliance with HIPAA and other funder requirements.
  • Collect all pertinent documents and complete intake and enrollment with all new clients.
  • Provide housing stabilization services including bi-weekly meetings; use of harm reduction approaches; linkage with community resources; identifying permanent housing options among other services. 
  • Assist with outreach in the community for new client referrals, as needed. 
  • Participate in multi-disciplinary team meetings as scheduled.
  • Participate in weekly individual supervisory meetings.
  • Other related duties as assigned.
